Saturday, august 9th at Kaimuki high school - two free sessions!

Free clinic open to keiki ages 6 to 13 all experience levels welcomed! Accomplished soccer coaches and the UH women's soccer team will train your keiki in soccer skills and good sportsmanship in a mentorship driven setting.

Ages 6-9

Parental Consent Waiver

I, the parent and/or guardian of the above-named applicant, hereby give my approval to said applicant's participation in Barefoot League’s Free Keiki Soccer Clinic. I do forever RELEASE, acquit, discharge, and covenant to hold harmless Raising Cane's, Kaimuki High School, and Barefoot League LLC, all coaches and affiliates of, any sponsors and their representatives, any volunteers and their representatives, from any and all liability from any and all actions, causes of action, and claims for any injury or damage, and any and all liability for any such injury or damage.

I hereby authorize the personnel of Raising Cane's, Kaimuki High School, and Barefoot League LLC to take any necessary steps for the required medical treatment for said child/ward. I hereby release Raising Cane's, Kaimuki High School, and Barefoot League LLC and its personnel from any and all liability for any medical treatment decisions made for the treatment of said minor child/ward.

I hereby give permission for my child's photo to be used in promotional materials and web promotions.
